APM delivers Personal Helpers and Mentors (PHaMs) employment services in Brisbane and Perth.
Funded by the Australian Government, APM's specialist PHaMs employment service team works directly with individuals with a lived experience of mental illness to provide practical support to help address issues in their lives that have been identified as barriers to gaining and maintaining meaningful employment.
We operate from a strengths-based recovery model and subscribe to providing a service which is client-focused, culturally appropriate and respectful of each individual's decisions to make informed choices.
APM encourages strong collaboration with all stakeholders, including participants, families, carers, referrers, mental health services and general community services, to ensure a seamless service.
PHaMs is currently transitioning to NDIS. As a registered service provider, we deliver:
- Life transition planning; including mentoring, peer support and individual skill development
- Individual skills development and training
- Individual social skills development
- Assistance with accommodation and tenancy obligations
- Support connection
- Coordination of supports
