If you have experience of mental illness we can help you address issues that may be preventing you from reaching your personal goals

APM delivers Personal Helpers and Mentors (PHaMs) services in Brisbane and Perth.

Funded by the Australian Government, our PHaMs service team works directly with people whose lives are severely affected by mental illness.

We aim to help people overcome social isolation and increase their connections to the community.

What PHaMs does

If you’re aged 16 or over and want practical support to explore your interests, strengths and abilities, we can help.

We help you set personal goals to improve your life and work with you to reach your desired outcomes.

You get a case worker who provides one-on-one support and works with you, your family (if desired), health services and other supports to help you:

  • build confidence in your own capabilities
  • develop better relationships with family and friends
  • manage everyday tasks you may find challenging
  • attend appointments and social activities
  • get involved in community activities, hobbies and sport
  • improve living arrangements

Our role with NDIS

PHaMs is currently transitioning to the National Disability Insurance Scheme (NDIS).

APM is not a provider of NDIS supports.

How to access PHaMs

There are a range of ways you can be referred to PHaMs

To be eligible, you need to be:

  • aged 16 or over
  • living with a severe mental illness that impacts your functioning and daily life
  • be willing to work towards goals in a recovery plan related to achieving new skills. These may include managing everyday tasks, improving health and fitness, participating in community, volunteering, training or finding a job
  • willing to apply to NDIS when this is available in your region

You can apply for PHaMs directly or through a referral by family or friends.

A range of community services, such as Disability Employment Service providers, clinical case managers, psychologists, drug and alcohol support services, or housing support services can also refer you to PHaMs.

We encourage strong collaboration with all stakeholders, including participants, families, carers, referrers, mental health services and general community services, to ensure a seamless service.

You’re in control of your support

We use a strengths-based recovery model for this service. This means we recognise you are in control of your recovery

We help you identify and build your strengths, and the steps to take to improve your life.

We help you benefit and grow stronger from your informed choices and actions.

Our service is focused on you and shaped by you. Our teams will recognise and respect your culture and be respectful of the informed choices you take.

We also work closely with any family members, health services, community supports or other support services you require, and only with your permission.
