We also set up regular health checks and treatment required, such as physiotherapy, occupational therapy and nursing services.

Our teams of friendly and experienced health professionals monitor your progress and will talk to you about using any other services that may speed up your recovery.

Rehabilitation in your home has many benefits to your recovery

When you can recover from an operation in your home, you have a say in the care you receive.

By being away from hospital you are less likely to experience surgery-related complications including hospital-borne infections.

We work with your care coordinator, doctor and health insurers to help deliver services you need in the six weeks after your surgery.

We ensure you get a fast, clear path of communication between your clinical specialists and your local GP.

You gain control over your recovery and focus your rehabilitation on returning to your daily activities sooner.

How to access APM @ home services

Before you go home, you will have an in-home rehabilitation plan in place. This includes identifying the services required and setting your rehabilitation goals.

Your care coordinator will assess your eligibility for the APM @ home service.

When we become your rehabilitation at home partner, we aim to visit you within 24 hours of being discharged.

Your first appointment with us includes making sure you have everything you need to stay at home. This includes:

  • Making sure you have a Welcome Home pack from your care coordinator
  • Reviewing your home modifications, equipment and aids to ensure they are suitable
  • Testing any aids or equipment to ensure they are working safely
  • Reviewing your treatment goals and expectations
  • Measuring your ability to move and carry out tasks
  • Notifying your care coordinator that your first session was carried out within one business day
  • We provide you with a report on your health and rehabilitation progress and make sure it goes to your GP and treating clinician.
