We understand how complicated it can be finding the right disability support

If you or someone you care for needs help accessing disability support our Local Area Coordinators (LACs) can help.

APM has been selected to deliver NDIS Partners in the Community (PITC) LAC Services in Darwin, and the Maryborough service area in Queensland, which includes Fraser Coast, Cherbourg, North Burnett and South Burnett.

Our LACs help people with disability, NDIS participants, families and carers to identify and access the support they need.

This includes helping you to realise your individual goals and aspirations by connecting you to community and mainstream services.

LACs also work with these services and organisations to be more inclusive and supportive of people with disability.

We are proud to provide LAC Services and our LACs are dedicated to helping you access the right support.

 

Let’s keep things simple

LACs work with you directly to identify and access the most suitable support services for you

We know everyone’s needs are different.

We also know how quickly your circumstances can change or become more complicated.

Your LAC will:

  • Meet with you, your family and carer to learn about your current situation

  • Make it a priority to understand your goals

  • Develop a plan to help you achieve your goals

  • Link you to information sources, supports and services in the community

  • As required, help you connect with the NDIA to determine your eligibility for the scheme

If you or someone you care for needs help accessing the National Disability Insurance Scheme (NDIS) or other support services in your community, we can guide you through the process.

The NDIA will contact you by letter to confirm your eligibility for the scheme.

Starting with the NDIS

Our LACs will help you with understanding what you need for accessing the scheme or developing your NDIS Plan.

 

An NDIA delegate is responsible for approving the funding attached to your plan. When your plan is approved, your LAC will help you put your plan into action.

Not eligible for the NDIS?

Not every person with a disability will be eligible for an NDIS Plan, if you or the person you care for need help, we can still support you with information and help to connect you to local mainstream and community organisations.

APM and the NDIA

The National Disability Insurance Agency (NDIA) has appointed APM to deliver LAC Services as part of the  Partners in the Community Program.

APM will no longer be an NDIS Registered Provider of Supports. APM will ensure it works with the NDIA so that no NDIA participant is disadvantaged during this exit process.

The NDIA deliver the NDIS to support Australians with disability.

Compliments, Feedback and Complaints

At APM, we invite you to provide us with feedback to help us improve our services.

Your comments will be treated confidentially, and all compliments, complaints and suggestions are taken seriously.

You can provide feedback on your experience by:

  • Speaking directly to your LAC
  • Contact APM’s local LAC manager
  • Email your feedback to: LAC@apm.net.au

If you make a complaint, we will resolve your issue as quickly as possible at the local level.

Our complaints resolution procedures require that we:

  • Take immediate action where there appears to be a high risk of harm, neglect or abuse
  • Acknowledge complaints within 24 hours of receipt
  • Call you within 48 hours of acknowledgement
  • Resolve complaints within 21 calendar days
  • Report information on our performance
