NDIS Local Area Coordination


Partners in the Community
As a NDIS Partner in the Community, APM Communities delivers NDIS Local Area Coordination services in parts of Western Australia, Queensland and the Northern Territory.
Our experienced Local Area Coordinators (LACs) work alongside people with disability, their families, and carers to help identify and access the supports they need.
Our Community Capacity Building team also helps empower local communities to become more inclusive and accessible.
They collaborate with local organisations and community groups, co-design community projects led by people with disability, and help educate the community about barriers people with disability can face.
What your Local Area Coordinator can help with
We understand that every person’s needs are different—and that life can change quickly.
That’s why our LACs take the time to listen and build a genuine understanding of your situation, goals and aspirations.
Your LAC can help you to:
- Learn about the NDIS, understand if you are eligible and apply for access
- Understand and use your NDIS plan
- Identify and connect with local supports and services
- Prepare for plan reassessments
- Access support during a change in circumstances
- Navigate the system when things become complex
We also have dedicated staff to assist people from First Nations, culturally diverse, and LGBTQIA+ communities.
Even if you're not eligible for the NDIS, our team can still connect you with community and mainstream supports that may assist.

Getting started with the NDIS
If you, or someone you care for, needs support accessing the NDIS, APM Communities can guide you through the process.
Our LAC’s will help you apply to the NDIS and gather the right information and evidence.
The NDIA will contact you to confirm your eligibly for the scheme. Once your plan is finalised and approved, your LAC will help you put your plan into action.
If you believe you meet the eligibility criteria for the NDIS and would like help to apply for access contact APM Communities.
Eligibility
You may be eligible for the NDIS if you:
- Are aged between 0 to 65
- Are an Australian Citizen, a permanent resident, or hold a special category visa
- Have a permanent disability that has a significant impact on your daily life
Not eligible for the NDIS?
Not every person with a disability will be eligible for an NDIS plan.
If you or the person you care for need help, we can still support you with information ad help to connect you to local mainstream and community services in your local area.

At APM Communities, our Local Area Coordinators (LACs) are skilled, knowledgeable, and ready to support you to navigate the NDIS with confidence.
Our LAC’s possess strong local knowledge of services and supports, along with an extensive understanding of disability, injury and illness.
Many are subject matter experts and people with lived experience, and all are committed to culturally aware and inclusive practices.
We’re proud to have teams across the country, supported by a network of experts who guide and strengthen our local delivery.

Creating a safe, enjoyable, and inclusive space
Inclusive disco group EasyBeatz has become more than just an event for people with disability – it has became a symbol of inclusivity and community spirit for clubs and organisations.
APM Communities helped set up a working group that included people with disability, to ensure their needs were met from the ground up.
And when the Byford and Districts Country Club were eager to better support inclusion in their community, EasyBeatz found themselves filling the dancefloor.
How a cricket dream came alive for Raz
NDIS Participant Raz lights up with pride as he describes how finally being actively involved in the sport he loves makes him feel alive.
Raz now plays cricket with a local WA club, St Marys, thanks to a collaboration between APM Communities and the Western Australia Cricket Association (WACA).
The team launched a 'come and try' short indoor cricket series to provide the opportunity to participate in an integrated modified cricket game designed by the WACA.
It was the perfect opportunity for Raz and other cricket fans to access the sport.
