APM has now received 100,000 referrals from My Aged Care.

The milestone is a huge achievement for APM's Regional Assessment Services (RAS) team who began operating in 2015.

Assessment Services General Manager, Jason Diederich, said it was a great achievement for the team and feedback from clients over the past three years had been positive.

“That's 100,000 older Australians who have been referred to APM to get support to remain independent in their own homes.

“And with a RAS client satisfaction rating above 95%, that's a lot of happy older Australians!” he said.

The RAS team delivers assessments to older Australians who are looking to stay in their homes with the help of local services for support.

The assessments are funded by the Australian Government as part of the My Aged Care program.

It enables eligible people to access a range of support services to meet their needs.

It can include a range of individual services or packages of support, such as:

  • meals and nutrition services
  • nursing, physiotherapy and other healthcare
  • help with transport simple home modifications such as hand rails
  • equipment such as walking frames
  • household cleaning
  • personal care such as help getting dressed
  • support to join social activities
